Student-Athlete Sports Medicine Information Page

Dear Student Athletes and Club Sports Members:

If you are expecting to participate in intercollegiate or club sport activities during the 2012-2013 academic year we will need from you a complete medical history, your primary insurance information and emergency contacts.

We have recently put into place a new and completely confidential way for you to send us your information.  The following link will access the Student Portal website www.NexttStudentPortal.com. The first time at the site you will be asked to “Create a new account.” To do this, you will need to enter our school code: SW458.  Once you have created an account, you can go back and edit information as needed.   Please enter all required information.  There you will also find forms that you will be required to read and electronically sign. Please read them carefully and sign them.  You must sign these forms to participate on an intercollegiate team or club.  This information must be completed by August 1, 2012.

In the event that you are injured while participating in an intercollegiate or club sport, it is important for us to know about any medical coverage you have to avoid delays in claim processing.  Swarthmore College provides secondary athletic accident insurance for our students participating in intercollegiate sports and club sport activities.  This coverage is in excess of your family and/or College health and accident insurance.

Injuries resulting from supervised practice or play in intercollegiate or club participation must first be reported to the Athletic Training Staff.  Claims resulting from those injuries will be submitted against your primary coverage first.  If you have any unpaid balances after a claim has been processed by your primary coverage, the athletic insurance will cover up to what is customary and reasonable.

We conduct orthopaedic screenings for all varsity student-athletes who have sustained an orthopaedic injury in the past twelve months and physical screenings for all juniors.  The dates will be determined later in the summer.
